Your First Visit

Please bring the following items to your first appointment:
Current and previous medical records: Any relevant records that help us understand your medical history and previous treatments. Digital copies of any imaging that may be pertinent to your visit such as X-Rays or MRIs with reports.
List of current medications: A comprehensive list of the medications you are currently taking, including dosages.
Insurance information: Please have your insurance card or details ready to ensure smooth processing.
